If you find these online forms are incompatible with your computer system, hard copies are available in the College Bookstore (Quad, Ukiah Campus, 1000 Hensley Creek Road; Lake Center, Lakeport; or Willits Center, Willits).

APPLICATION FORM- PDF Online Automated Form
After completing the form online, print it for your and your supervisor's signature, then click on the "email" button to submit an electronic version. You must turn in the signed "hard copy" to the CWE office (either hand deliver it; fax it to 707-468-3120 for Willits or Ukiah, or 707-263-1908 for Lake; or mail it to the appropriate Center) by the required date. IF YOU HAVE A DIAL-UP (SLOW) CONNECTION, BE PATIENT. IT MAY TAKE A FEW MINUTES FOR THIS FORM TO DOWNLOAD.
