The Mendocino College Supplemental Admissions/Change of Information Form can be used to request the following changes and corrections:  Name, Address, Telephone Number, E-mail Address, Date of Birth, Social Security Number, Degree Earned, Major and Educational Goal.  The completed form should be forwarded to the Office of Admissions and Records in Lakeport, Ukiah or Willits.  Please note, Social Security Number changes can only be processed in the Office Admissions and Records - Ukiah.  Click here to access the Mendocino College Supplemental Admissions Form.
